1. Constitution

Breakaway was formed as a charitable organisation to advance the wellbeing of people with a disability, through alignment with the United Nations Convention on the Rights of Persons with Disability (2006) and Australia’s human rights statutory framework.

Our Constitution is Breakaway’s governing document, last updated April 2023.

2. Code of Conduct

Our Code of Conduct signals our collective commitment and promise to each other, participants and customers. It provides guidance to all our employees on how we maintain trust, do things right and act in a manner that we say we will do.

3. Privacy and Confidentiality Policy

Breakaway understands that your privacy is important.

Our Policy sets out how we protect your privacy and manage your personal information. It applies to all people we interact with, including our participants and customers and employees.

4. Whistleblower Policy

Our Whistleblower Policy includes information about whistleblowing, and how Breakaway will respond to and investigate disclosures and protect whistleblowers.

5. Complaints Policy

Breakaway is committed to ensuring that any person or organisation using services provided by Breakaway or affected by its operations has the right to lodge a complaint and to have their concerns addressed in ways that ensure access and equity, fairness, accountability and transparency. Properly handled complaints result in improved organisational reputation.

This policy should be read in conjunction with the Whistleblower Policy.

6. Service Agreement Terms

Our Service Agreement Terms provide our participants with clear details about what they can expect from a Breakaway service. The Terms set out our commitment to participants and the participant’ obligations and rights. They reflect a person-centred approach with language in line with NDIS Practice Standards. Terms are signed the first time a Service Agreement is put in place with us, or when they are updated. An Easy English version is available here.
